Statement on My Federal Case

A direct, first-person statement by Dr. Sonny Saggar, MD

In 2024, I pleaded guilty in the U.S. District Court for the Eastern District of Missouri to one count of conspiracy to commit healthcare fraud. In 2025, I was sentenced to 35 months in federal prison and ordered to pay restitution in the amount of $742,528. This page provides the factual context of my plea and my perspective as a physician leader.

Operational Context

The billing operations for my clinics were managed by an external, third-party billing company. During a period of three and a half years, the billing company handled our submissions without raising notifications or concerns to me or the clinic leadership regarding potential compliance errors or supervision documentation discrepancies.

Regardless of the third-party billing company's operational role, I recognize that as the owner and medical director, the ultimate responsibility for verifying compliance, billing accuracy, and physician supervision documentation standards rested with me. My plea represents my acceptance of responsibility for those supervisory and administrative failures.

For Fellow Physicians

The complexities of collaborative practice agreements and supervision documentation are substantial. For any physician managing clinics or supervising assistant physicians, maintaining direct, active oversight of billing practices and verifying that documentation meets every regulatory requirement is crucial. Relying entirely on external billing services to flag issues is a vulnerability that can lead to systemic errors.
